Search 'fire alarm certificate' and you get the NICEIC shop at £23 a certificate, five companies who will come and test your system for a fee, a PDF you can buy, and one free form — a manufacturer's paper certificate for its own smoke alarms. Nowhere on page one can a fire alarm engineer pick the certificate the job needs, type in the system and the tests, and print it.
This form does that. It is not a copy of the BSI model certificates or of any scheme's pad — it is FieldHive's own layout on the content the standard's certificates record — and it prints as a branded A4 PDF.

Which certificate, and for what
Non-domestic (BS 5839-1:2025, or 2017 for older work) or a dwelling (BS 5839-6). Then the type: design, installation, commissioning, acceptance, verification, inspection and servicing, or modification. Quick starts set all of that for the usual jobs — a rented home at Grade D1 LD2, a small commercial L2 system, the six-monthly service.
The system and the tests
Category or grade, control equipment, zones and loops, device counts, mains and standby supplies, the extent covered, any variations from the standard. Then the checks for that certificate type — every device tested, sound levels, cause and effect, standby capacity, documents handed over — with the faults found and the outstanding items.
Declaration and next service
Your company and registration number, the competent person's signature on screen, an optional second signature for the responsible person, and the next inspection date filled in from the interval — six months under BS 5839-1.
Seven certificates under BS 5839-1, one under BS 5839-6
BS 5839-1:2025 — the non-domestic code, in force since 30 April 2025 and replacing the 2017 edition — gives a model certificate for each stage: design, installation, commissioning, acceptance by the user, verification, the periodic inspection and servicing visit, and (new in 2025, Section 7) modification of an existing system. Where one organisation did the design, installation and commissioning of a small system, one combined certificate is usual; where different firms did each stage, each signs its own.
In a dwelling, BS 5839-6:2019+A1:2020 uses a single certificate of design, installation and commissioning for Grade C, D and F systems, stating the grade (A, C, D1, D2, F1, F2) and the category (LD1, LD2, LD3). The same tool produces it — choose 'Dwelling' and the fields change.

Weekly
The user operates a manual call point during working hours, a different one each week, so every call point is tested in turn — and records it in the log book.
Six-monthly
BS 5839-1 recommends inspection and servicing by a competent person at intervals not exceeding six months, with every automatic detector tested within twelve months. The inspection and servicing certificate — the fire alarm test certificate — records the visit; choose it as the certificate type and the template becomes that form.
False alarms
The 2025 edition asks the user to investigate false alarms and the commissioning or handover organisation to say how — the certificate has a box for the information provided.
Dwellings
Under BS 5839-6 the occupier tests the alarms with the test button at least monthly; the certificate records that the instructions were supplied.
Sits with the emergency lighting certificate, the job sheet and the invoice
A fire alarm visit often tests the emergency lighting too, and both certificates go in the log book. The job sheet records the visit, the invoice bills it, and where the wiring was altered the minor works certificate covers the electrical work. FieldHive has a free form for each.
