Search ‘minor electrical installation works certificate’ and you get the NICEIC pad for £23, a fire-safety shop selling the same pad for £19.95, the IET’s model form as a PDF to print and handwrite, and three articles explaining when you need one. Nowhere on page one can you type the client, the circuit and the test results and get a completed certificate.

Five sections, the same order as the standard
A — Describe the minor works
Client, date completed, installation address, what was done, any departures from BS 7671 or permitted exceptions (with the risk assessment attached), and comments on the existing installation.
B and C — Earthing, bonding and the circuit
Earthing arrangement (TN-S, TN-C-S PME or PNB, TT, TN-C, IT), Zdb, main protective conductors; then the DB, circuit, reference method, conductor sizes and the OCPD, RCD, AFDD and SPD details.
D and E — Test results and the declaration
Continuity, ring continuity, insulation resistance, polarity, Zs, RCD time and the test-button checks. Choose A4:2026 or A2:2022 in the declaration, sign on screen, download.
For an addition or alteration to an existing circuit — nothing bigger
A Minor Electrical Installation Works Certificate covers electrical work that does not extend to the provision of a new circuit: an extra socket-outlet or lighting point on an existing circuit, a relocated switch, a replaced accessory or luminaire. A new circuit, or a replaced consumer unit or distribution board, needs an Electrical Installation Certificate instead. Whatever the size of the job, the circuit is inspected and tested, and one certificate is issued for each circuit worked on.
The certificate is required by BS 7671 (Regulation 644.4), and the person who signs it is certifying that the work was designed, constructed, inspected and tested to the standard. Since 15 April 2026 that standard is BS 7671:2018+A4:2026; it is mandatory for new work from 15 October 2026, and this form defaults to it with A2:2022 available for work completed before that date.

Choose the standard the work was tested to. New work from 15 October 2026 must reference A4:2026.

The certificate is the electrical record. The job sheet records the visit, the invoice bills it, and the certificate of completion is the commercial sign-off with the customer. The four together are the paperwork for a small electrical job, and FieldHive has a free form for each.

The person named in Section E is certifying the work under BS 7671, and where the work is notifiable under Part P of the Building Regulations, notification is a separate duty this form does not discharge.
