Search 'daywork sheet' and you get six sheets other contractors uploaded to Scribd, a generic form builder that records hours but not rates, a rail possession sheet, an app listing and two social posts. Nowhere on page one can a subcontractor type in the crew, the plant and the materials, apply the percentage additions, and print a sheet the QS will sign.
This sheet does that. Labour, plant and materials in three tables that add up, the prime cost and the percentage additions summarised the way the RICS definition describes them, and two signature boxes with the words that make the second one easy to get.

Contract and instruction
Your business, the contractor or client, the contract and the site, the instruction or variation reference and who gave it, the date, and what was done and why it is daywork.
Labour, plant and materials
One card per operative — name, trade, hours, rate. Plant with own or hired, hours and rate. Materials with quantity, unit and rate. Each table totals itself. Switch pricing off and the sheet prints as a record of resources and hours only.
Percentages and signatures
The percentage additions for labour, plant and materials from the subcontract; the summary shows prime cost, additions and the daywork total excluding VAT. Sign on screen, and get the contractor's representative to sign before the end of the day.
Instructed work valued on time and materials, not at contract rates
Daywork is the method of valuing work on the basis of the time spent by the contractor's operatives, the materials used and the plant employed — used when instructed work cannot be measured and priced at the rates in the contract: unforeseen obstructions in the ground, opening up, work of a kind the bills never priced. The daywork sheet is the daily record of what was used, and the valuation is prime cost plus a percentage for incidental costs, overheads and profit.
Under most subcontracts the percentages are the ones tendered on the daywork schedule; where none were agreed, the RICS definition of prime cost of daywork applies — labour, materials and plant at prime cost, each with an agreed percentage addition. The sheet takes the percentages you enter and applies them per category; it invents none.

On the day
Get the site manager, engineer or QS to sign before the end of the day. Every other argument about a daywork claim can be had later; a missing signature usually cannot.
What the signature means
The contractor's representative confirms the resources and the hours — that the people, plant and materials recorded were used. It does not confirm the rates or that the work is properly daywork. The sheet says so, which is why they sign it.
One per day
One sheet per day per instruction, numbered. The next-day button keeps the crew, the plant and the rates and clears the hours, so the second sheet takes a minute.
With the application
Submit the signed sheets with the weekly or monthly application for payment, against the instruction reference. Keep copies.
