Search 'pat testing form' and you get a PDF someone uploaded, a training academy's log sheet, the IET's forms index, an Excel register, a register generator that builds a spreadsheet of column headings, and a certificate you fill in by hand afterwards. Every one of them makes you write the appliance list twice — once in the register, once on the certificate — and none of them works out when the next test falls due.
This form takes the list once. Each line is an appliance with its class, the formal visual inspection, the readings and the result. Switch the preview and the same list becomes the certificate for the client; switch back and it is the record sheet for your register. It also says on the form what HSE says: there is no legal frequency, and the interval is your judgement.

The visit
Your company and name, the client and site, the test date, the instrument you used and the interval in months you recommend for this equipment in this environment — the next test date fills in from it. The record number moves on each visit.
The appliances
One line each: an ID for the label, what it is, where it lives, Class I or Class II, the formal visual inspection, the earth continuity (Class I only), insulation resistance and leakage readings, and the result. IDs count up as you add lines; a new visit to the same site keeps the list and clears the readings.
Record sheet or certificate
The record sheet is A4 landscape with every reading; the certificate is one A4 sheet with the summary, the failed items called out, the declaration and your signature. Download either as a free PDF; the editable Word versions and your logo unlock with an email.
No legal frequency, no legal record — a duty to maintain, and a competent tester
The Electricity at Work Regulations 1989 require electrical equipment to be maintained so as to prevent danger, so far as is reasonably practicable. HSE's guidance is explicit that the regulations do not make inspection or testing a legal requirement, do not set a frequency, and do not require equipment to be labelled or records to be kept — records are a management tool, and a good one. The frequency depends on the type of equipment and the environment it is used in: a power tool on a building site needs looking at far more often than a kettle in an office.
The IET Code of Practice for In-service Inspection and Testing of Electrical Equipment (5th edition, 2020) is the industry reference for what to inspect, what to test, the pass values and suggested initial intervals. The person doing the testing must be competent — trained, with the right instrument and able to interpret the results. This form records what a competent person did; it does not make anyone competent.

Formal visual first
Plug, fuse, flex, strain relief, casing, signs of overheating. A failed visual is a fail; the electrical tests are not needed to condemn it.
Class I
Earth continuity 0.1 Ω plus the resistance of the lead; insulation resistance at least 1 MΩ. Earthed equipment — metal-cased kettles, extension leads, most site tools.
Class II
No earth test (there is no earth). Insulation resistance at least 2 MΩ. Double-insulated equipment — the square-in-a-square symbol.
Leakage
Where the instrument measures it or a substitute test is used; record the value and the instrument's own pass limit.
